If you you placed your order from a healthcare facility and are dissatisfied for any reason and wish to return an unused, unaltered product, please fill out the form below. We will contact you within 24 hours and issue a full refund upon receiving and inspecting the returned item. Unused products can be returned from a healthcare facility for up to 30 days upon receipt.
If you placed your order directly and are dissatisfied for any reason and wish to return an unused, unaltered product within 15 days of receiving it, please fill out the form below. We will contact you within 24 hours and issue a partial refund (minus our shipping cost and a 15% restocking fee) upon receiving and inspecting the returned item.
